Individuals may call 800-246-5743 to schedule an appointment for any of our locations in Lincoln, Langlade, or Marathon counties.  When calling to schedule an appointment, staff will ask questions to determine if you are eligible for the program.  Information you will need to provide:

• Number in family
• Birthdates for those to be enrolled in the program
• Anticipated due date if pregnant
• Current Income
• Address
• Telephone number that WIC can reach you at
• After potential eligibility is determine, staff will offer you the earliest appointment available.

IN PERSON

Individuals may come to a WIC clinic any day it is open and apply for WIC.  For those who do not mind waiting to be worked into the schedule, certification can be completed at the time of this visit.  You will need to bring:

• Children under 5 years who are to be certified
• Identification for parent and children – social security cards cannot be used
• Proof of Income – check stubs, child support notification, income tax forms
• Proof of residency – any mail with current address
• Proof of pregnancy – if pregnant woman is to be certified
